1. Fill out the form below and submit
2. Once your form is received you will either:
- Be sent a page with information for submitting payment
- Or, if you are a Unite for Her participant, you will be contacted by phone to collect credit card information.
3. Every participant will be asked to fill out a medical history form (HIPAA compliant), a waiver of liability form, and submit dietary preferences.
